Provable Forms
Forms with proof built in
Build a form, publish it, and every response is checked against the published version, hashed, and sealed on the BSV blockchain. Respondents keep a receipt. You keep a record nobody can quietly edit.
Every response sealed
Each submission is validated on the server against the exact form version it answered, then Merkle-hashed and anchored on BSV. Anyone can fetch its seal from a public endpoint.
A receipt for the respondent
After submitting, respondents see the hash of their answers and can download a JSON receipt โ their own copy of what they sent.
Versioned forms
Publishing freezes a version, and the sealed record shows which version each response used. Republish in one step or roll back to an earlier version.
Build fast
A visual builder with 23 field types โ signatures, file uploads, repeat groups, ratings โ plus conditional logic and multi-step flows. Start blank, from 33 ready-made templates, or from building blocks.
Put it anywhere
Share a link, drop in an iframe, or add the embed script, which resizes to fit. Only sites on your allowlist can embed.
Connect your systems
Signed webhooks fire when a response arrives and when it is sealed, with retries and a delivery log. Export to CSV or JSON, or drive everything from the REST API with an API key.
How it works
- 1Build
Design the form, or start from a template.
- 2Publish
The form version is fixed and sealed.
- 3Collect
Respondents fill it in by link or embed, and each gets a receipt.
- 4Seal
Every response is validated, hashed, and anchored on BSV.
- 5Use
Review responses, export them, and receive webhooks with the seal details.
